(b) An insurer or rating organization may not:
(1) Monopolize or attempt to monopolize, or combine
or conspire with any other person or persons, or monopolize the
business of insurance [or] OF any kind, subdivision, or class
thereof;

All policies of credit life insurance, credit health, and
involuntary unemployment benefit insurance shall be delivered or
issued for delivery in this State only by an insurer authorized
to do an insurance business therein and shall be issued only
through holders of [qualifications licenses or authorizations]
LICENSES OR CERTIFICATES OF AUTHORITY OR QUALIFICATION issued by
the Commissioner.

(a) In this section "nurse anesthetist" means a registered
nurse who has been certified by the Council on Certification or
the Council on Recertification of Nurse [Anesthetist]
ANESTHETISTS as a nurse [anesthetists] ANESTHETIST.

(b) The items set out in subsection (a)(3) of this
[subsection] SECTION need not be stated in the sequence or order
in which they appear. Additional items may be included to
explain the computations made in determining the amount to be
paid by the insured.

Notwithstanding any provision of a group or individual
policy or contract, including profit and nonprofit health plans,
or any certificate issued thereunder, of health, sickness,
accident, or disability insurance, delivered or issued for
delivery within the State, whenever such policy, contract, or
certificate provides for reimbursement for any service which is
within the lawful scope of practice of a health care provider
duly licensed under the Health Occupations Article, the insured,
or any other person covered by the policy, contract, or
certificate, shall be entitled to reimbursement for such service.
Nothing contained in any form for payment under a group or
individual policy or contract [or] OF disability insurance,
delivered or issued for delivery within the State, shall preclude
a duly licensed health care provider from attesting to the
rendition of service which is within the lawful scope of practice
of the attesting provider. The provisions of this section apply
