            I hereby certify that the bearer hereof a black
       man by the name of Charles Forard about
       twenty three years old five feet nine inches
       high having a scar over his right eye as
       small scars in the middle and little fingers
       of the left hand as a Freeman being the
       descendant of a Freewoman by the name of
       Sarah Ford wholeved in the family of
       Benjamin Wood Deceased where the said
       Charles Ford was raised
